Who’s Eligible?
- Students aged 16 to 21 with a disability or special learning needs
- Many participants receive ORS (Ongoing Resourcing Scheme) funding for additional school support
How It Works:
- Signed Agreement – We team up with you, your family, and school to plan your transition.
- Personalised Transition Plan – Set goals and create a roadmap for success.
- Brokerage Services – Get connected to the right resources, programs, and work experience.
- Final Report – Review achievements and celebrate your progress.
